My full name is Ashok Damji Thakkar.
I am 65 years old.
My date of birth is 6th April, 1958.
I am residing at 1A/13, 3rd Floor, Matru Shraddha CHS, Near Tilak
Nagar School, Tilak Nagar, Dombivali (E), Dist. Thane Maharashtra-421 201.
I have studied upto Matriculation.
My wife expired 9 months before i.e. in the month of March 2023.
My son is residing with me in my residential premise. He is major and doing job in travel agency in “Booking.com”. He is earning Rs.30,000/- to 35,000/- p.m. salary.

I started my first Proprietorship in the year 2006. The said Proprietorship carrying on supply of Food products in Hotels. The business premise was situated at Cabin No. M-11, on Mezzanine Floor, Steel Yard House, Sant Tukaram Road, Iron Market, Masjid (E), Mumbai 9. In 2017, the same was converted into Partnership Concern till January 2023. I was earning 10 lakhs annually as the turnover of the business was more than 1 Cr each year.
I undertake to produce deed of Partnership.
I have two Current Accounts in the Kotak Mahindra Bank, Crawford Market Branch and HDFC, Crawford Market Branch and one Saving Account in ICICI, Masjid Bunder (E) Branch. I do not have any Fixed Deposits, Life Insurance Policies. I have Mediclaim of Star Health India Assurance for Rs. 5,00,000/- (Rupees Five Lakhs Only), premium for the same is Rs.30,000/- p.a.
I undertake to produce bank statements & copy of mediclaim
policy
I don’t have any ancestral property.
Goods vehicle is in Partnership firm name, which was purchased on
loan from AU Small Bank. I don’t have any personal vehicle. I had driving license which was expired in 2020.
I undertake to produce papers regarding loan for puchase of
vechicle.
I don’t have any Jewellery.
I don’t have passport.

I had started business in the year 2006 and started borrowing loan
in 2007-08 from Aditya Birla, Bajaj Finance as well as from known persons namely Mr. Vinod Gadia, Mr. Bipin Mehta, Mrs. Jagruti Mehta & Mr. Ritesh Mehta. Business was closed down due to Covid-19 lockdown.
I undertake to produce all the Loan Agreement.
I dont have any personal savings.
I have balance in my saving bank account is Rs.10,000/-
I have debit card of HDFC, ICICI Bank as well as credit card of
Kotak Mahindra Bank, but right now, I am not using those cards.
All undertakings are accepted.
