"Admitted in Common (Trauma)/trauma/TRPolytrauma (113) admission No.10122126172, dt. 20.09.2022 time 2.37 A.M., CR No.1012215346309 in SMS Hospital, Jaipur due to Head Injury and weakness of left upper and lower limb. Patient discharged on 01.10.2022, From Neurosurgery unit-I ward TRNeurosurgery, ward-112, Patient admitted due to assault on dated 19.09.2022 as History given by attendant of Patient. Diagnosis Head Injury with Right Frontal Parital Bone Fracture with Right Frontal Pneumocephalis with Right Parital Subarrachnoid hemorrhage with Right clavicle fracture with Right Parital subdural-Hemorrhage.
On dated 21.09.2022 NCCT Brain done and finding says AIR underlaying Right Frontal Bone and in Interhemispheric fissure seen.
Subdural hemorrhage noted in Right high parital lobe, convexity maximum thickness approx 10 mm.
Point 1. Pt. ID.092201130138 dt. 21.09.2022 Depressed fracture of Right Parital Bone is seen. Subdural Hemorrhage noted in left frontal lobe convexity, Maximum thickness approx 4 mm.
Point 2. on dated 23.09.2022 again NCCT BRAIN done Pt. ID- 092201144283.
Communicated displaced fracture seen in parital Bone right sided with underlying subarachnoid hemorrhage and multiple air foc in Right fronto parital lobe Region.
Point 3. on dated 28.09.2022, Pt. ID 092201178707. again NCCT BRAIN done. Localised ill defined hemorrhagic contusions with surrounding significant edema seen in Right Parital lobe outward displaced fracture of Right Parital Bone seen.
Point 4. NCCT cervical dorsal lumber spine says no abnormality seen.

Point 5. As per BHT Record shows weakness of left side of Body due to assault.
Opinion:All above mention findings and thoroughly study of BHT of MR. Sanjay S/o nandkishore admitted in SMS Hospital Dt.20.09.2022 to 01.10.2022 Injury No.1 & 4 is grievous in nature and danger to life and Injury No.4 is inflacted due to head injury No.1 and object to created the injury No.1 is sharp so we have to consider injury No.4 wapen is also sharp."
