Issue of Death Certificate by Medical Practitioners to Informants

WITH WW GOVERNMENT OF INDIA 13 WW MINISTRY OF HOME AFFAIRS OFFICE OF THE REGISTRAR GENERAL, INDIA dated the Octoberfvs, 2002 Circular No.3/2002 it has been observed that there is some confusion about to whom the certificate of cause of death is to be issued by the Medical Practitioners and who will submit such certificates to the registrars. In this connection, it may be clarified that under the provisions oi Section 10(3) ol the Registration of Births and Deaths Act,1969, the certificate of cause of death has to be issued by the MedioaI Practitioners to the informants responsible for reporting the events of biiths and deaths under section 8 or 9 of the Act who in turn will deliver the same to the registrar alongwith the death report form, Further it has been observed that in most of the cases of institutional deaths the cause of death certificates and the related report forms are sent to the Registrars independently AlsoIn some Ia?es the cause of death column in the death repcIIt form is left blank even when it is mentioned in cause of death certificate As cause of death certificateIS detached for independent processing; It Is necessary to ensure that the underlying cause of death recorded in cause of death certificate is also noted in the death report form. Similarly it has been noted that all the causes of death given in the cause of death certificates are IepIoduced together in the death report from making it difficult to identity the exact underlying cause of death after the forms are sepaIated. this results in inconsistent data on cause of death in areas where MCCD Is in operation it is requested to kindly take note of the above points and inform all concerned to take appropriate precaution in this regard as suggested above.
