Accident Register extract issued by Sanjay Gandhi Institute of Trauma & Orthopaedics, Bangalore and the other medical reports are also produced by the petitioner. To substantiate his case, the injured examined himself as PW-1 and he has clearly deposed with regard to the accident. One Singaraiah, Technician working in Sanjay Gandhi Hospital is examined as PW-2. The alleged accident took place on 14.04.2012. Ex.P12 is the accident register extract, issued by the Sanjay Gandhi Institute of Trauma & Orthopaedics, Bangalore stating that one Hanumanthe Gowda, aged 45 years is admitted to the hospital with a history of road traffic accident. Further, it reveals that the patient was suddenly hit by another two wheeler at around 3.45 p.m. on 14.04.2012. Ex.P5-wound certificate issued by the Rajiv Gandhi Accident Hospital, & Research Institute, Byrasandra, Bangalore also reveals that the petitioner Hanumanthe Gowda is aged 45 years, admitted to the hospital through his brother, Mr.Thamanna with a history of road traffic accident. That, around 3.45 p.m. on 14.04.2012, the doctor has examined the petitioner and found injuries as shown in Ex.P5 i.e., deep abrasion on the occipital region and swelling on the face.
